Athletic Department Player Forms


ALL PAPERWORK MUST BE RETURNED AND FULLY COMPLETED PRIOR TO PARTICIPATION AT ANY LEVEL, IN ANY PRACTICES - NO EXCEPTIONS

  1. Each student athlete must have a completed WIAA Sports Physical Card on file at WEHS. Note: Physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year. 2020-2021 sports physicals must be dated 4-1-2019 or later.
  2. WIAA Alternate Year Cards are to be completed every other year. 
  3. Parents & Athletes must read the High School Athletic Code Handbook and submit the completed Athletic Code Signature Form & Parental Insurance Waiver once per school year
  4. An Agreement of Coaches Expectations e-signed and dated by parent/guardian & student for each sport that you participate in is required.
  5. WIAA required Concussion Form needs to be completed once per school year.
  6. An Emergency Information Card must be submitted to have on file for medical needs. This card must be completed prior to participation in the first sports season of the year for each student athlete. 
  7. Activity Fee Payment ($50 per sport, $75 for Lacrosse is required for each sports season. Envelopes for payment are available in the business office or athletic office. These payments can be turned in to either office or directly to the coach.
  8. A parent or guardian must attend ONE Athletic Department Sports Meeting per school year. The Winter Sports Meeting will be held on Tuesday, October 28th at 6:30 p.m. in the Auditorium. Please make sure to sign in at the meeting.
